The shoulder surgery that costs a fraction in China
Shoulder problems are among the most common reasons adults lose function — and they're among the most expensive to fix in the United States.
A torn rotator cuff, a worn-out shoulder joint, or chronic impingement can steal your ability to reach, lift, and sleep comfortably. When conservative treatment fails, surgery is often the answer. And in the US, that answer carries a heavy price.
A rotator cuff repair runs $15,000 to $25,000 in the US without insurance. A total shoulder replacement runs $20,000 to $52,000.
In China, the same procedures at top-tier hospitals cost $4,000 to $7,000 for rotator cuff repair and $6,000 to $14,000 for shoulder replacement — a saving of roughly 60-75%, before travel costs.
And for many patients, China's high-volume orthopedic centers offer something just as valuable as the savings: surgeons who perform these procedures every day.

Why shoulder surgery costs less in China
The same structural economics that drive every Chinese medical advantage apply here: lower surgeon fees, government-regulated facility charges, lower hospital overhead, and malpractice costs that are a fraction of US levels.
The surgical technique is identical to the West. Chinese orthopedic surgeons use the same arthroscopic equipment, the same suture anchors, and the same shoulder implant systems (like Zimmer and DePuy) found in US operating rooms.
What's different is volume and price structure. China's top orthopedic centers perform shoulder surgery in very high volume, and the implant and consumable costs are kept low by centralized procurement. The result: the same procedure, at a fraction of the US price, from surgeons who do it constantly.
The real price comparison
Prices below reflect typical all-in costs at reputable Chinese hospitals, compared with average US billing. All figures are in US dollars.
| Procedure | China | United States | Savings |
|---|---|---|---|
| Arthroscopic rotator cuff repair | $4,000 – $7,000 | $15,000 – $25,000 | 60-75% |
| Total shoulder replacement | $6,000 – $14,000 | $20,000 – $52,000 | 60-75% |
| Shoulder arthroscopy (debridement) | $3,000 – $6,000 | $10,000 – $20,000 | 65-70% |
| Subacromial decompression | $3,500 – $6,500 | $12,000 – $22,000 | 65-70% |
The shoulder replacement gap is especially large. In the US, self-pay shoulder replacement can exceed $50,000 depending on the hospital and implant type. In China, even with a premium implant, the all-in package rarely exceeds $14,000.
After adding flights and a couple of weeks of accommodation, a China shoulder surgery trip still comes in at less than half the US price for the procedure alone.

Rotator cuff repair vs shoulder replacement: which do you need?
These are very different procedures for very different problems.
Rotator cuff repair — repairs a torn tendon in the shoulder's rotator cuff, usually done arthroscopically through small incisions. The surgery takes 1-2 hours, and recovery involves a sling for 4-6 weeks followed by months of physiotherapy. Best for active patients with a repairable tear.
Total shoulder replacement (arthroplasty) — replaces the worn joint surface, like a hip or knee replacement but in the shoulder. Takes 2-3 hours, with a sling for several weeks and rehabilitation over months. Best for advanced arthritis where the joint surface is worn out.
Reverse shoulder replacement — a specialized implant for patients with massive rotator cuff tears and arthritis, where the normal anatomy can't support a standard replacement. More complex and usually reserved for older patients.
Shoulder arthroscopy (debridement/impingement) — a minimally invasive "clean-up" for impingement, frayed tissue, or minor tears. The least invasive and fastest recovery.
Your surgeon will determine the right procedure from imaging and exam. The key question for a torn rotator cuff is whether the tear is repairable and how fresh it is — the earlier you treat it, the better the outcome.
Where to go: China's top shoulder surgery centers
Shoulder surgery in China is best at the same high-volume orthopedic centers that lead joint replacement:
Beijing Jishuitan Hospital (积水潭医院) — China's benchmark orthopedic center, with a dedicated sports medicine and shoulder program and enormous annual volume.
Shanghai Sixth People's Hospital (上海六院) — another orthopedic powerhouse with strong shoulder and sports medicine departments.
Other leading centers include Peking Union Medical College Hospital (协和), Huashan Hospital (华山), and Ruijin Hospital (瑞金).
Private international hospitals — English-speaking staff, direct billing, and pre-booked appointments, at a higher price point.
For shoulder surgery specifically, look for a hospital with a dedicated shoulder/sports medicine department rather than a general orthopedics ward — the focused volume matters for outcome.
What the procedure and recovery look like
Rotator cuff repair and shoulder replacement are performed under general anesthesia, often combined with a nerve block for post-operative pain control.
The timeline for an international patient:
| Day | What happens |
|---|---|
| Day 1-2 | Pre-op workup: MRI review, X-rays, anesthesia consult |
| Day 3 | Surgery (1-3 hours depending on procedure) |
| Day 4-5 | Discharge with sling and pain medication |
| Day 7-10 | First follow-up, then fly home |
| Weeks 4-6 | Sling period; passive physiotherapy begins |
| Months 3-6 | Progressive strengthening and range of motion |
Most patients need 10-14 days in China total. You'll fly home in a sling with a detailed physiotherapy protocol, and ideally a coordinated rehab plan with a physiotherapist at home.
The honest caveats
First, the tear type and timing matter more than the destination. A repairable rotator cuff tear treated early has an excellent outcome; a chronic massive tear may not be repairable at all. Get a clear diagnosis and a written surgical plan from a home orthopedic surgeon before you travel.
Second, recovery is a marathon, not a sprint. Full shoulder recovery takes 3-6 months, sometimes longer. The surgery is 2 hours; the rehabilitation is the real work. Commit to the physiotherapy protocol.
Third, rehabilitation access matters. You'll need months of physiotherapy after surgery. Confirm your home rehab plan before you leave — this is the difference between a good and a great outcome.
Fourth, "all-in" needs verification. A legitimate quote separates surgeon, anesthesia, facility, implant (for replacement), and physiotherapy. Confirm exactly what's included.
